(a) “Eligible Vitality Plan” means any VitalityHealth Personal Healthcare plan (i.e. excluding any business/corporate VitalityHealth healthcare plans) or any VitalityLife life insurance plan purchased directly from Vitality and no other third party broker or intermediary;
(b) “Our”/“Us”/“We”/“Vitality” means Vitality Corporate Services Limited trading as VitalityHealth and VitalityLife;
(c) “Promotion” means Vitality’s “Refer A Friend” promotion which these Terms and Conditions specifically apply to, as further detailed below;
(d) “Qualifying Plan” means an Eligible Vitality Plan which has: (a) been purchased by a Referred Member as a result of a Referral; and (b) has been in force with all premiums paid up to date, for at least three consecutive months immediately following the start date of the Eligible Vitality Plan. For the avoidance of doubt, where an Eligible Vitality Plan is cancelled or suspended due to non-payment of premiums within the first three months of commencement, this will not class as a Qualifying Plan for the purposes of this Promotion;
(e) “Referred Member” means an individual who:
(i) is referred to Vitality by a current holder of an Eligible Vitality Plan;
(ii) as a consequence of this referral, purchases a new Eligible Vitality Plan directly from Vitality, either online at vitality.co.uk or over the phone. Referrals are not valid if the new plan is purchased through a third party; and
(iii) meets the conditions set out in clause 4.
(f) “Referring Member” means an individual holder of an Eligible Vitality Plan purchased directly from Vitality either online via vitality.co.uk or over the phone, and who has referred a Referred Member to Vitality by nominating them either by sharing a unique link via social media, or by entering the Referred Member’s email address and clicking ‘Share’ on the Refer a Friend page on the Vitality Member Zone. For the avoidance of doubt, a Referring Member does not include any person that:
(iv) has made a referral in their capacity as a professional financial adviser, authorised representative or insurance intermediary; or
(v) has purchased their Eligible Vitality Plan through a professional financial adviser, authorised representative or insurance intermediary.
(g) “Referral” means a Referring Member has shared a unique link, either by email or social media with a Referred Member, from the section dedicated to the Promotion within the Referring Member’s Vitality Member Zone.
(h) “Referral Reward” means either one (1) John Lewis gift voucher or one (1) Amazon gift voucher, as selected by the Referring Member, and each to the value of £100 (Pounds Sterling).
(i) “Referred Member Reward” means either: one (1) John Lewis gift voucher to the value of £100 (Pounds Sterling) or one (1) Amazon gift voucher to the value of £100 (Pounds Sterling). For the avoidance of doubt, a Referred Member will only be eligible for a maximum of one Referred Member Reward.
(j) “Reward” means the Referral Reward and the Referred Member Reward, either collectively or as appropriate. All Rewards are subject to the provider’s terms and conditions. (k) “Successful Referral” means the instance where a Referred Member has purchased a Qualifying Plan as a result of a Referral; and
2. Subject to these Terms and Conditions, all Referring Members will receive a Referral Reward for each Successful Referral they make during the Promotion.
3. Once a Referral qualifies as a Successful Referral, Vitality will send both the Referring Member and Referred Member an email stating that the Referred Member’s Qualifying Plan has been activated. Vitality will email again after the third monthly premium payment of the Referred Member’s Qualifying Plan has been collected, and this email will enable the Referring Member and Referred Member to claim their selected Reward via a secure link on the Vitality Member Zone. Once the Reward has been claimed, a second email containing a link to a PDF version of the chosen gift voucher will be sent. The Reward must be claimed from Vitality within six months of the start date of the applicable Referred Member’s Plan.
4. A Referred Member will be eligible to receive a Referred Member Reward where the below conditions are met. The Referred Member must have:
(a) purchased a Qualifying Plan;
(b) registered for this Promotion (by clicking on the unique link shared by email from the Referring Member, asking them to confirm their name and email address and accept the terms of this Promotion) before purchasing their Qualifying Plan;
(c) entered their Vitality membership number via the email link provided, within 60 days of the start date of their Qualifying Plan; and
(d) validated their Vitality membership number on the section dedicated to the Promotion within the Vitality Member Zone within 90 days of the start date of their Qualifying Plan.
5. The Referred Member will be notified by email of their eligibility for a Referred Member Reward within four weeks of paying the third premium for their Qualifying Plan;
6. Subject to clause 7, all Rewards are not transferable, non-exchangeable and cannot be redeemed for cash.
7. In the event that a Reward is unavailable due to circumstances beyond reasonable control, Vitality reserves the right to substitute it for another Reward of similar value.
8. This Promotion does not apply to new members added to an existing Vitality plan.

10. The Referral Reward will be available via an email link sent to Referring Member and Referred Member within two months of the Referred Member’s third premium being paid, so please allow time for due diligence checks and processing.

14. We reserve the right to suspend or cancel transactions and reallocate Rewards if the referral (for which a Reward has been given) is suspected of being fraudulent and in such circumstances the Referring Member or Referred Member may have their details passed to the relevant authorities for further investigation.
15. We accept no liability for lost entries and no responsibility for difficulties experienced in submitting an entry to this Promotion, including any technical, hardware or software failures of any kind or lost or unavailable network connections which may limit or prohibit an individual’s ability to participate in the Promotion. Wherever needed to contact the Referring Member or Referred Member via email, it is your responsibility to provide us with a valid, active email address, and Vitality cannot be held responsible for an invalid email address or other problems relating to any delay to the delivery of or non-receipt of emails, including the effect of spam filters.
16. To the maximum extent permitted by law, Vitality accepts no liability for any loss, damage and/or personal injury suffered by participating in this Promotion and/or arising out of the enjoyment of a Reward.
17. We reserve the right to alter, amend or terminate the Promotion at any stage without prior notice. This Promotion is administered by Buyapowa Limited and XEXEC on behalf of Vitality.
