Confirm a Vitality activity
There are some activities that we can’t track, like a visit to the dentist or your GP. If you’ve had a check-up or test that’s listed in the Vitality points guide and meets the eligibility criteria, send us evidence of the activity and we’ll update your Vitality points statement.
Use our online form to submit evidence.
- Evidence could be a copy of your results, a copy of your receipt with the doctors name and address and details of the check-up or test.
- You need to send us evidence within six months of the activity so you don’t lose your points.
- Our screening and fitness assessment partners will send us your results so you don’t need to. Please check at your appointment.
